HOPE (Hospital Outreach Program in Education) is a free follow-up service for 6-12 students attending Department of Education Schools.
HOPE plays a vital role in supporting students and their families who experience serious illness and chronic health conditions that disrupt their school attendance.
Our team work alongside the student, family, census school and medical professionals and develop a wholistic program. Our aim is to work on the student's wellbeing for a positive transition back to school.
